  • A copy of your invoice with the following information:
    • Contractor's Name
    • Installation Address
    • Make(s) and Model(s) of the Installed Unit(s)
    • Date of Installation
  • Details about the old equipment removed and the new equipment installed 
  • Pre-Approval Confirmation
  • Customer Details
  • Equipment Details
  • Review and Submit

If you have been pre-approved you will be asked for your confirmation number
This confirmation number would have been shared with you via email after submitting the pre-approval form.
